Mozilla Firefox instructions for downloading Form RP-5217-PDF
- Select Download Form RP-5217-PDF below.
- Select the hamburger menu
in the top right corner of your browser to check the status of the download in the Mozilla Library by selecting downloads.
Note: If your settings allow, Mozilla Firefox may try to open Form RP-5217-PDF in your browser window when complete, but this will not have the necessary 2D barcode. Do not use a version of the form without a 2D barcode. - When the download is complete, select the Show in folder icon
to open your computer's Downloads folder. Form RP-5217-PDF should be the most recent item in that folder (labeled rp5217). - Right-click on the form from the Downloads folder, select Open with, and select Adobe Acrobat Reader. Ensure this version has the 2D barcode necessary.
- Complete the form.
